Federal Custody Death Reporting Forms | Bureau of Prisons Data Improvement Improving Data Collection on Federal Custody Deaths After reviewing the current data on deaths in federal custody, I found that the information provided was lacking and, frankly, questionable. Shockingly, 10% of these deaths are reported as occurring for unknown reasons, and another 10% are documented as happening in unknown locations. Such ambiguity raises serious concerns about transparency and accountability within the federal system. To address these gaps, I developed a new three-form system designed to close the data collection gap and provide more precise insights.
