Federal Custody Death Reporting Forms

Federal Custody Death Reporting Forms | Bureau of Prisons Data Improvement
Improving Data Collection on Federal Custody Deaths

After reviewing the current data on deaths in federal custody, I found that the information provided was lacking and, frankly, questionable. Shockingly, 10% of these deaths are reported as occurring for unknown reasons, and another 10% are documented as happening in unknown locations. Such ambiguity raises serious concerns about transparency and accountability within the federal system.

To address these gaps, I developed a new three-form system designed to close the data collection gap and provide more precise insights. This updated system will assist in identifying and addressing the challenges faced by federal inmates and facilities more effectively. Each form is tailored to specific roles within the Bureau of Prisons, ensuring comprehensive and accurate data collection on these critical incidents.

Federal Custody Death Reporting Form for Corrections Officers
Form specifically for corrections officers to document and report fatalities accurately.
Federal Custody Death Reporting Form for Wardens
Form specifically for wardens to provide comprehensive data regarding fatalities in custody.
Federal Custody Death Reporting Form for BOP Central Office
Form for the Bureau of Prisons Central Office to manage data and reports on custodial fatalities nationwide.
